Laparoscopic Pyeloplasty is a type of reconstructive surgical procedure that repairs the narrowing or scarring of the ureter (a tube that attaches the kidney to the bladder) through a minimally invasive procedure. Pyeloplasty is a combination of two words, ‘Pyelo’ and ‘Plasty’ where Pyelo refers to the renal pelvis or kidney and ‘Plasty’ refers to a surgical procedure that repairs, restores, or replaces.

Laparoscopic Pyeloplasty is usually performed by a urologist or general surgeon to correct Ureteropelvic Junction (UPJ) obstruction. The advantage of pyeloplasty is that it has the highest success rate compared to other surgical options for Ureteropelvic Junction Obstruction (UPJ). It also has a good long-term success and is effective 85% to 100% of the time.

Ureteropelvic Junction (UPJ) obstruction is a type of condition where a blockage can occur at the junction of the ureter and renal pelvis (kidney).

The ureter is a long, thin tubular structure that carries urine produced by the kidney to the urinary bladder. It actively propels urine from the kidney down into the bladder. This process of transportation is known as peristalsis.

When the Ureteropelvic junction obstruction occurs, it results in the decreased flow of urine down the ureter with an increase of fluid pressure in the kidney. The increased pressure affects the function of the kidney and may deteriorate it over time.

The Ureteropelvic junction obstruction can be congenital or may develop over time owing to infection, scar tissue, kidney stones, previous treatments or surgeries, etc.

Signs and symptoms of Ureteropelvic Junction Obstruction

Ureteropelvic obstruction may cause sharp pain in one side of the body between the upper belly area. It may occur at regular intervals and might increase after the consumption of alcohol, or excessive fluids. The pain is not restricted to the abdomen, it may spread down to the groin. Sometimes, people may confuse the Ureteropelvic obstruction pain with the normal pain.

Other symptoms can be

  • Abdominal mass
  • Urinary tract infection
  • Vomiting
  • Poor growth in infants

Causes of Ureteropelvic Junction Obstruction

The majority of conditions like Ureteropelvic Junction Obstruction are present since birth because of the poor development at the foetal stage. Sometimes, family history could also become a reason.

  • Kidney stones
  • Urinary tract infections
  • Narrow ureter opening
  • Unusual folds in the inner wall of the ureter
  • Unusual/Accidental twisting of ureter
  • Previous surgeries

Diagnosis

An ultrasound can be performed to detect a Ureteropelvic Junction Obstruction. Further imaging tests can also be performed as and if required. Sometimes, this condition is detected during regular evaluations like X-rays or ultrasound.
